Sample Answer 1: 

Introduction: Last year, I had scheduled an appointment with a dentist for a routine check-up.

Explanation of the situation: I regrettably had to cancel the appointment due to heavy traffic. I was going to leave early, but I didn't realise how long it would take to get to the clinic. It took me over an hour to get to the clinic because of the abnormally high traffic that day. The dentist had already left by the time I got there. I was very angry with myself for being so thoughtless, and I felt so ashamed. 

Outcome: I had to change the appointment, and the earliest time slot was two weeks away. I discovered that when making plans to steer clear of situations like these in the future, it's critical to take unforeseen occurrences into consideration.

Sample Answer 2: 

Introduction: A few years ago, I had a job interview with a reputed company.

Explanation of the situation: I was eager to take advantage of the chance and had studied hard for the interview. I entered the wrong day on my calendar, though, and I missed the appointment. The appointment had long passed by the time I realised my error. Both heartbroken and ashamed, I was. 

Outcome: I apologised for my error as soon as possible to the interviewer over the phone. Fortunately, the interviewer was accommodating and consented to reschedule the interview for the following week. I discovered the value of verifying every detail and making a note of any crucial information.
