The test lasts two days. 

1st Day

  • Around 232 multiple-choice questions
  • Made up of 6 groups of 38–39 items each 
  • Each block has 60 minutes. 

2nd Day:

  • Around 180 multiple-choice questions sSpread out over six 45-minute blocks.
  • A 7-minute CCS instruction is also included .
  • Following that are 13 case simulations, each with a maximum of 10 or 20 minutes of real-time.

Around 7 hours, including 45 minutes of break time and a 5-minute optional instruction.

2nd Day:

It takes around 9 hours and includes a 5-minute optional instruction.

Step 3

Must have completed Step 1, Step 2 CK, and Step 2 CS. Must meet eligibility requirements set by the medical licensing authority where the applicant seeks licensure.

USMLE Sample Questions: Types & Examples

Step 1

One of the most common formats for step 1 is a Single-Best-Answer Question. It is linked to a single question with four or more response possibilities (for example, A, B, C, D, and E). You can go through the USMLE Step 1 practice questions to gain a better understanding of the exam.

Several questions require you to interpret graphic or graphical materials. You must choose the best response to the question. Other possibilities may be partially correct, but only one best solution exists. This is the most common and standard multiple-choice question structure on the exam.

Step 2

For Step 2, these are the most common formats:

  • Patient scenarios can be presented in either a Vignette (paragraph) or Chart/Tabular format for any Single Item or Sequential Item Set. The chart/tabular form of test items is intended to mirror patient charts but not to be an exact copy of a patient chart. 
  • A patient-centered vignette is linked to a single question with four or more response possibilities.
  • A sequential item set can be linked to two or three questions about the information delivered. Each question is related to the first patient scenario but tests a different point. 
  • The abstract item format comprises a summary of an experiment or clinical inquiry provided in a way that a physician is likely to encounter, such as an abstract accompanying a research report in a medical journal.

Step 3

Patient scenarios, pharmaceutical commercials, and abstracts are among the multiple-choice question forms in Step 3.

Computer-based case simulations are interactive simulations of a patient-care situation designed to assess your clinical management approach, including diagnosis, treatment, and monitoring.

USMLE Step 1 Sample Questions
